The aircraft was lost in a fatal accident on 6th of September 1968. It crashed in the water just south of Huvudskär, in Stockholm's outer archipelago, during a personnel transport mission from Gotska Sandön to the mainland. There were 12 people onboard and only two survived. Four of the crew and six passengers lost their lives. The cause of the accident was determined to be a mechanical malfunction in the rear rotor hub.
The aircraft was eventually replaced by Y-64 (04064).
